Child must be enrolled by parent or legal guardian.
Parent will need photo ID.

Kindergarten Registration--We will do a brief screening.
Please bring the following items to registration:

statement/lease/rental agreement

One current utility bill
-- power, water or gas --no phone bills) (Must show name and
address)

Certified birth
certificate --Copy will not be accepted

Immunization Form -- #3231 (available from pediatrician or
Health Department)
Vaccines required

3 Hepatitis shots
-
2 MMR shots
-
2 Varicella (chicken pox) or history

Polio and DPT -- last vaccine must be given at 4 years of
age or later. If last vaccine given before 4th birthday, child
must have another vaccine.

Vision, Hearing and Dental Form #3300 (available from
pediatrician or Health Department)

Social Security Number -- The parent/guardian may choose not to
provide a Social Security number by signing a written waiver
provided by the school.

Photo ID of enrolling parent
